Intake and Application

Thank you for showing interest in our program! We are glad you have come this far and are considering joining HOM. The first thing to do is to come in and fill out an application. Please call during open hours, Monday through Friday from 8:00 AM to 5:00 PM to talk to a counselor and make an appointment.

We know it can be difficult to make that call and we do our best to make you feel comfortable on the phone - make the call right now and take your first step!
